Services covered
NR-32 concerns healthcare services and health-promotion, care, recovery, research and teaching activities within the scope defined by the regulation. Worker protection is not confined to clinical personnel. Cleaning, maintenance, laboratory or waste-management staff may face significant exposure conditions. Assessment must follow actual tasks, rather than professional category or the unit’s name alone.
Identifying exposures
The regulation sets requirements for biological risks and considers agents and exposure sources, transmission routes, service characteristics and existing measures. The PGR must reflect the facility’s situations. Stating that the whole hospital has biological risk is insufficient: tasks, areas, procedures and contact possibilities must be distinguished, including handling materials outside the place where they originated.
Connection with occupational health
The PCMSO must consider relevant risk information and plan responses to accidental exposures, with designated people and care resources. Vaccination aspects under the applicable framework are also included. A preventive response pathway must enable the person to know where to go and who activates assistance. Storing the protocol in a folder inaccessible during the shift does not demonstrate that it can be implemented.
Beyond biological risk
NR-32 also addresses chemical hazards, radiation, waste, cleaning and other service conditions. Patient safety and worker safety are related but are not the same assessment. An effective disinfectant may require occupational exposure controls, storage arrangements and training. Selecting a measure requires consideration of both the care process and those preparing, applying and removing it.
Shared-task example
After a procedure, cleaning personnel receive a room containing improperly segregated materials. The problem is not resolved merely by reminding them to wear gloves. The handover, the placement of objects and who checks completion of the preceding activity must be reviewed. This hypothetical situation illustrates an interaction between teams that needs organisational action as well as individual controls.
Review and documentation
Changes in techniques, equipment or areas may alter exposures and require review of preventive information. The entry on sharps injuries expands on one risk but does not represent the whole of NR-32. Records must respect confidentiality and allow investigation of working conditions without disclosing clinical data. Specific obligations require consultation of the Brazilian regulation and related provisions.
