Hepatitis A and occupational exposure

Hepatitis A is an acute liver infection transmitted mainly by the faecal–oral route. Occupational prevention considers faecal material, care tasks, food handling and hygiene conditions, without confusing it with hepatitis infections transmitted primarily through blood.

Distinguish it from other hepatitis infections

Hepatitis A may cause tiredness, digestive symptoms and jaundice, but asymptomatic infection also occurs. It does not produce the chronic infection associated with some other hepatitis viruses. Hepatitis describes liver inflammation, not a single transmission route. A procedure focused exclusively on needles and blood therefore does not by itself prevent an agent usually acquired through ingestion of material contaminated with the virus.

Assess tasks rather than job titles

Consider nappy changing, assistance with toileting, bathroom cleaning, faecal-material handling and contaminated-water work where relevant. Identify how contamination could reach hands, food or the mouth. Employment in a particular sector does not establish identical exposure for its entire workforce. Assessment should describe actual duties and hygiene conditions, including occasional operations and maintenance that may differ substantially from the usual working routine.

Interrupt the faecal–oral pathway

Provide handwashing with soap and water, clean eating areas and procedures for cleaning and removing protective equipment. Gloves suitable for a task do not authorise subsequent handling of food or clean surfaces. In care settings, integrate standard precautions with toileting arrangements. Check whether staff can follow clean and dirty routes during an ordinary shift, including periods of increased workload or reduced staffing.

Base vaccination on assessment

Vaccination can prevent hepatitis A, but occupational indication should consider risk, history and applicable health recommendations. It does not follow automatically from a job title and does not replace hygiene. Vaccination records support later contact decisions. The schedule and any measures after exposure belong with the healthcare service; a general recommendation from another country does not by itself determine an individual worker’s management.

Report a case promptly

Transmission may occur before jaundice or recognisable illness appears. Report an identified case or relevant contact so that the situation and contacts can be assessed promptly. Do not wait for colleagues to develop confirmed symptoms. People with pre-existing liver disease or other clinical circumstances need specific assessment, without the employer requiring access to medical details unnecessary for implementing agreed preventive measures.

Investigate without automatic attribution

Collect duties, dates, shared activities and possible contact with contaminated material. A case in a worker does not demonstrate occupational origin, and several cases do not automatically identify the responsible food or facility. Coordinate with public health when appropriate and retain information needed for investigation. Preventive learning should produce practical changes to hygiene, pathways or training while keeping epidemiological conclusions separate from initial suspicions.
