Presenteeism: working with health problems

In one of its most widely used meanings, presenteeism describes working while ill or experiencing a health problem. It can occur on site or remotely. Preventive analysis focuses on conditions that make recovery or working in a health-compatible way difficult.

In brief

Presenteeism does not simply mean staying for many hours or performing poorly. Nor is all work during recovery harmful. Distinguish pressure to work without suitable conditions from a planned, adapted return with appropriate professional support.

Clarify what is being measured

The literature uses different definitions: some describe attending while ill and others focus on the effect of health status on work. Before comparing data, explain the definition, period and collection method.

Presenteeism cannot be inferred from time records. Nor does lower productivity establish that an illness exists. Address the phenomenon using relevant information without turning a manager’s impressions into diagnoses.

Which conditions may encourage it

The OSHwiki review identifies factors such as job insecurity, financial pressure, high demands, limited autonomy and difficulties covering for someone. Team expectations and supervisors’ attitudes also influence it.

A culture that celebrates working while ill can make needs for support harder to communicate. The preventive response is therefore not limited to recommending personal habits: it must review psychosocial factors and work organisation.

Why reducing absenteeism is not enough

A reduction in workplace absenteeism does not, by itself, demonstrate improved health. It may coincide with better conditions, but also with greater pressure not to be absent. Indicators need to be interpreted together.

Economic estimates of presenteeism also depend on different assumptions and methods. Using well-defined internal data to decide measures is preferable to assigning a universal cost to every day worked with symptoms.

Distinguish pressure from adaptation

A planned return to work may include adaptation of tasks or conditions. That differs from demanding normal performance without considering limitations and recovery needs.

Whether work can continue is assessed according to the situation and job demands, with relevant healthcare support. Remote working does not automatically resolve the problem either: working from home can maintain the same pressures and make disconnection harder.

What the organisation can review

Provide channels for communicating difficulties, train managers, review workloads and plan cover. Adjustments must be real: reducing one task while maintaining incompatible deadlines may not relieve pressure.

Follow-up must respect health data protection. Aggregated trends and observed barriers are usually more useful for improving organisation than collecting personal diagnoses. Team participation helps reveal obstacles that attendance records do not show.

Hypothetical example: a team without cover

People in a team report continuing to work with health problems because nobody can cover their tasks. The company reviews distribution of knowledge, support and absence planning.

It then checks whether measures allow needs to be communicated and the planned support to be used. The objective is not to increase or reduce an attendance figure in isolation, but to prevent the organisation from effectively forcing people to work under unsuitable conditions.
